Millcreek Health Center is a privately owned, 61-bed, state and federally certified, long-term care facility operating under the direction of a licensed physician.  Millcreek Health Center provides the following:

· Comprehensive 24-hour nursing care by registered nurses,  licensed practical nurses and certified nursing assistants

· Social services

· Alzheimer’s and dementia care;   WanderGuard  alert system (to prevent unwanted departure)

· Physical therapy, occupational therapy, speech and hearing services

· Medicaid application assistance

· Full monthly calendar of entertainment and   activities at and away from facility, under the supervision of a licensed recreational therapist

· Housekeeping,  laundry  and maintenance   services

· Transportation to and from appointments and activities outside our facility

· Dietary services with all diets monitored by a registered dietitian.  Three meals served daily with restaurant-style seating;  between meal and evening snacks offered

· Sunday Church Services

· Beauty/barber shop open one day per week
